Intake Form

This is where you provide your personal, health, and insurance information so we can verify benefits before you arrive. This will also make your visit run more efficiently.

Acknowledgement Signatures

These are forms you will need to sign before your visit to acknowledge you were offered a copy of our Notice of Privacy Practices and that you agree to our Financial Policy, both are linked at the bottom of the page.

Release of Records (optional)

This form can be utilized to have your records transferred from another doctor or facility to Peek Eye Care.
